What you will do:
Assess and evaluate psycho-social status.
Develop the plan of care in consultation with the physician and other health care team members.
Provide interventions directed toward enhancing the patient’s and family’s ability to cope. Direct patient and family to social programs and supportive instruction and counseling.
Assist physician and other health team members in understanding the significant social and emotional factors related to the health problems of the patient.
Observe, record and report to the physician and/or supervisor the patient’s reaction to treatment and any changes in the patient’s condition.
Evaluate the home environment and makes the appropriate community resource recommendations.
Advise, counsel, and when appropriate instruct family in patients psycho-social needs.
Participate in utilization review, team conferences and in-service education.
Prepare and submit, as directed, all case documentation, including clinical and progress notes, and required payroll information according to Agency policy.
Utilize and provide patient / family with appropriate community resources as needed.
Provide services within the Agency’s catchment area.
Supervise care provided by Bachelor’s Degree level social workers.
Consult and coordinate services with other disciplines.
Participate in discharge planning.
Acknowledge patient rights including privacy and confidentiality.
Consistently display a professional appearance.
Interact with internal and external customers using positive and professional approach.
Assume responsibility and display reliability for the completion of all tasks, duties, communications and actions.
Maintain professional growth and development through networking, membership in professional organizations and continuing education.
Participate in performance improvement activities.
Abide by Agency policy and procedures.
Accept other duties as assigned.
Skills, education, and experience:
Master of Social Work degree from a school of social work accredited or approved by the Council on Social Work Education.
Current CPR, TB and health clearance and core competency per skills checklist.
At least one year of experience in social work in a health care setting.
