We are looking for a Home Care Specialist/Sales, to join our Private Duty Department in our Thousand Oaks Branch.


The base pay range for this position at commencement of employment is expected to be between $33-36 per/hour; however, base pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience.

There is also an additional commission structure based on success in the role!

Home Care Specialist is a professional representative of the Agency who functions as a home care educator and intermediary between the Agency and clients/families, hospitals, facilities, other professionals and the community.  The purpose of this role is to facilitate an uncomplicated transition of clients to caregiver services consistent with the mission and goals of the Agency. The position serves as part of the home care team with time spent performing inside sales, traveling to prospective client’s residence and the community.

What you will do:
  • Screens and facilitates referrals timely including but not limited to HUB referrals, inquire calls via phone or email
  • Follows-up on all referrals as appropriate including: set appointments, mail information, perform follow-up phone calls, email and texts as appropriate. Documents real time status and maintain accurate records of activities in appropriate software/app
  • Meet /coordinate with the prospective family/client on all Starts of Care: perform a home safety evaluation to assess hazards in the residence, sign consents, develop care plan, obtain deposit, and ensure secure payment arrangements
  • Consults with Program Manager, Staffing Supervisor or Branch Manager to ensure appropriate and safe utilization of caregiver services as needed
  • Promotes a positive working relationship serving as liaison between the client/family, staffing coordinator, human resource coordinator, field caregivers, referral sources and third-party payer as appropriate
  • Develops and maintains relationships with, and identifies needs of community organizations, discharge planners, case managers, physicians, hospitals, nursing facilities, medical groups, home health & hospice agencies and other care professionals who have an opportunity to utilize Agency services
  • Coordinate with the home health and hospice department and other sales specialists to identify appropriate clients to assist in securing appropriate services and follow-up support.
  • Maintains a level of knowledge of Agency services to present to community care providers and payor sources
  • Attends and participates in rehabilitation, discharge planning and client care meetings as requested
  • Provides and/or coordinates agency sponsored educational opportunities and presentations within the community
  • Provides appropriate education and marketing materials to targeted and active accounts to effectively promote utilization of Agency’s services
  • Participates in sales meetings to ensure compliance with operational practices and appraisals of pertinent community related events
  • Demonstrates effective negotiation and closing techniques
  • Responsible for achieving monthly goals as assigned in the individual Annual Sales Commission plan on a consistent basis
  • Prioritizes, organizes and manages the workload to meet performance targets
  • Acknowledges clients’ rights including privacy and confidentiality
  • Assumes responsibility and displays reliability for the completion of all tasks, duties, communications and actions.
  • Maintains professional growth and development through networking, membership in professional organizations and continuing education.
  • Participate in performance improvement activities.
  • Abides by Agency policy and procedures.
  • Accepts other duties as assigned
Who you are:
  • High School Diploma is required; Associate’s Degree or higher preferred
  • At least 2 years of client service or sales experience, home care industry experience is preferred
  •  Goal oriented and performance driven
  • Must possess excellent organizational skills and be detail oriented
  • Must demonstrate strong written and verbal communication skills and be able to interact effectively with health care professionals, clients, peers and management
