This is a field based position requiring visits to skilled nursing facilities and/or homes. Most employees perform 2 days in field and 3 days home working on reports and referrals. Mileage reimbursed.

Position Overview
The Case Manager I supports members by conducting clinical assessments, developing individualized care plans, and coordinating services across medical, behavioral, and social needs. This position is 100% remote but requires residency in Bernalillo, Sandoval, or Valencia County, with up to 40% travel for in-home member assessments.
Key Responsibilities
Complete timely clinical assessments and determine eligibility for case management services
Develop and implement individualized care plans in collaboration with members, caregivers, providers, and support systems
Conduct telephonic and in-person (home) visits to assess progress and evaluate care plan effectiveness
Monitor ongoing member needs and update documentation to reflect interventions and outcomes
Promote integration of medical, behavioral health, and long-term care services
Facilitate interdisciplinary care team (ICT) meetings and collaborate with peers across disciplines
Apply motivational interviewing techniques to support member engagement and goal achievement
Identify and address barriers to care through coordination and resource linkage
Travel up to 40% within assigned counties to perform in-person assessments
Required Qualifications
Must reside in New Mexico – specifically in Bernalillo, Sandoval, or Valencia County
Bachelor's degree in social work, psychology, public health, or a related field
(OR) at least 1 year of direct care coordination or case management experience
1–3 years of experience in case management, disease management, managed care, or medical/behavioral health settings
Valid driver’s license with clean driving record and reliable transportation
Must be available full-time, Monday–Friday from 8:00 AM to 5:00 PM MST

RN licensure is not required for this position
Behavioral health and social service backgrounds are highly valued
