Job Description – Peritoneal Dialysis or PD Travel RN (Registered Nurse)
  • Provide direct patient care and education for peritoneal dialysis patients in an outpatient (chronic) clinic setting.
  • Assess, plan, implement, and evaluate individualized nursing care plans.
  • Train and support patients and caregivers on performing PD at home, including equipment use, aseptic technique, and troubleshooting.
  • Monitor patient progress, dialysis adequacy, and treatment compliance through clinical assessments and lab results.
  • Collaborate with nephrologists, dietitians, social workers, and other members of the care team to optimize patient outcomes.
  • Perform and document ongoing patient assessments, including vital signs, weight, access site condition, and fluid balance.
  • Respond to patient concerns and complications promptly, escalating issues when necessary.
  • Educate patients and families on managing ESRD, medications, nutrition, and infection prevention.
  • Maintain accurate and timely electronic health records in accordance with facility and regulatory standards.
  • Ensure compliance with all federal, state, and local regulations, as well as organizational policies and procedures.
  • Participate in quality improvement initiatives and staff development activities.

Job Requirements
  • 1+ year of recent experience as a home hemodialysis (HHD) RN
  • Graduate from an accredited nursing school
  • Current state specific nursing license in good standing, or if applicable, a compact state RN license in good standing
  • BLS certification required (Chronic and Acute settings)
  • ACLS certification required (Acute setting only)
